Ucare is an independent, not-for-profit and dynamic health plan providing health coverage and services to Minnesotans of all ages. Working in partnership with health care providers and community organizations, Ucare offers: • Ucare Choices and Fairview Ucare Choices – Commercial products for individual and family coverage available through MNsure. • Ucare for Seniors – Medicare Advantage products for individuals eligible for Medicare. • Minnesota Senior Health Options (MSHO) – Integrates Medical Assistance and Medicare services and payments for people age 65 and older. • Minnesota Senior Care Plus (MSC+) – For people eligible for Medical Assistance age 65 and older. • Ucare Connect (a.k.a. Special Needs BasicCare, or SNBC) – For adults with certified disabilities (physical and/or mental illness, certified by state or federal government) ages 18-64 (may remain in SNBC when they turn 65). • EssentiaCare – A new Medicare Advantage product offered in partnership with Essentia Health for Medicare-eligible people in 10 north-central Minnesota counties. • MinnesotaCare and Prepaid Medical Assistance Program – Income-based Minnesota Health Care Programs for individuals and families in Olmsted County. This service will grow May 1, 2017, when Ucare begins offering MnCare coverage in 55 counties and Medical Assistance in 38 counties. We`re located in restored warehouse offices in Northeast Minneapolis near major freeways, public transportation sites and convenient retail centers. Our diverse workforce is dedicated to serving Minnesotans in all ages and stages of life with high-quality health care coverage products. For the 7th year in a row, employees voted Ucare a Star Tribune Top 150 Workplace, including recognition for workplace excellence. Ucare also received a special Communication excellence award. We`re the only company to have earned this honor twice since the rankings began in 2010.
